چکیده

Background Pressure loss estimation based on the simplified Bernoulli equation frequently misclassifies the severity of aortic stenosis. Consequently, several investigators have on the basis of fluid dynamics theory derived pressure loss indices aimed at improving the clinical approach to pressure estimation [1-3]. However, CMR may offer a stronger alternative. The primary cause of pressure loss in aortic stenosis is dissipation of turbulent kinetic energy (TKE) into heat. New CMR methods permit direct estimation TKE [4]. We sought to evaluate the relationship between CMR-measured TKE and previously described pressure loss indices.

Evaluation of myocardial fibrosis with cardiac magnetic resonance contrast-enhanced t1 mapping in adults patients with aortic stenosis

Background Development of diffuse myocardial fibrosis in aortic stenosis (AS) has been associated with an adverse prognosis. Measurement of extracellular volume (ECV) from T1 mapping contrast magnetic resonance (CMR), has been recently validated as a non-invasive method for assessment of such diffuse myocardial fibrosis. Our objective was to evaluate the functional consequences of diffuse myocar...

Assessment of Aortic Stenosis Severity by Cardiovascular Magnetic Resonance

Cardiovascular magnetic resonance (CMR) has become a valuable tool to corroborate aortic stenosis (AS) severity when echocardiography assessment is discordant. Moreover, CMR can provide useful complementary information about AS severity and hemodynamic markers. In particular, the use of advanced 4D flow CMR allows a comprehensive assessment of complex flow alterations produced by AS. This revie...

Pre- and post-contrast T1 mapping for detection of myocardial fibrosis in severe aortic stenosis before and after aortic valve replacement

Background Severe aortic stenosis (AS) leads to diffuse fibrosis in the myocardium, which could cause poor clinical outcome. Cardiovascular Magnetic Resonance (CMR) with recently T1 mapping is a promising technique to identify diffuse myocardial fibrosis. The Objective of the present study was to assess myocardial fibrosis by CMR using T1 mapping in patients with surgical AS and evaluate the ev...

Assessment of aortic stenosis severity by rest CMR correlates well with stress echocardiography in the setting of low left ventricular flow states

Background Identifying the high-risk subset of patients with severe aortic stenosis (AS) and low transvalvular pressure gradient due to significant left ventricular (LV) impairment has relied on the assessment of the haemodynamic changes in response to flow normalization using pharmacological stress. Although cardiovascular magnetic resonance (CMR) aortic valve area (AVA) derived from planimetr...
